Incoming Resident Forms

NOTE: You will need to fill out both the Application for Assignment to Housing and the Sex Offender Disclosure & Acknowledgement Form and send them in to get on the waiting list for housing. If you prefer to print the forms and sign instead of using the electronic signature option, click here to open the PDF versions of these forms. Then you may fax it to us at (402) 502-4557 or scan and email it to us at offutt@afpmanagement.com. The other attachments are resources for you to familiarize yourself with if you would like.

Application for Assignment to Housing (PDF)
You must be active duty with a Common Access Card (CAC) to be able to complete the form with electronic signature.  You can fill and save your information to this form.  Complete Blocks 1 - 21 of the form.  In Block 21, Remarks, please annotate the following  "I give the Housing Office permission to release my information to Rising View."  Go to Signature of Applicant (Block 22), click the box to digitally sign.  Your electronic signature and the date will be entered on the form.  When complete, save the form to your computer and e-mail with a copy of the orders that brought/are bringing you to Offutt AFB to randilynn.hill@offutt.af.mil and william.spain@offutt.af.mil at the Offutt AFB Housing Management Office. Note: If you do not yet have a copy of the orders bringing you here, send the application and sex offender disclosure first and send a copy of the orders as soon as you receive them.

Sex Offender Disclosure & Acknowledgement Form
You must be active duty with a Common Access Card (CAC) to be able to complete the form with electronic signature.  You can fill and save your information to this form.  Click on the underlined portion at the top of the form to type your full name.  Go to the bottom of the form and click on "Click to Sign," select the middle button "Sign," a box pops-up, select your signature identity and then click "OK."  Your electronic signature and the date will be entered on the form.  When complete, save the form to your computer and e-mail to randilynn.hill@offutt.af.mil and william.spain@offutt.af.mil at the Offutt AFB Housing Management Office.
